I think code reviews are very important and useful when time is spent  
on them. We use them extensively at Mozilla, and a great deal of  
issues are caught before they ever hit a repository or worse, a  
production system.

I don't know that we're at a point where we need to enforce a priori  
review, but having a system in place to support a more structured  
review system would be nice, I have to say.
