 I tried to go through their process, however they require something beside
 what's listed above.

 I contacted Chris (see above) if he's has a "checklist/preparation sheet"
 so all this information could be prepared offline and then the submission
 process could be completed without wondering "what would be the right
 information for this bit?"...
